High keeps images up to 3000 pixels on their longest side and is best for printing. Medium, up to 2000 pixels, suits most uses. Small, up to 1400 pixels, makes the smallest file, which is handy for emailing or uploading to a form with a size limit.
The images are stored in the PDF as JPG pictures, which cannot be see-through, so transparent areas are filled with white.
